Permission-based by design
You choose whether Apple Health is part of the story.
Health Diary asks for access through Apple’s HealthKit permission flow. You can decline access, grant selected categories, or change permissions later in iOS settings. Health Diary does not write medical diagnoses into Apple Health.
Disconnecting Apple Health removes that source from future plans; the diary and goal-based experience remain available.
Apple Watch

Apple Watch can contribute activity and sleep data to Apple Health. The Health Diary watch app then shows your Today suggestion, supports quick mood check-ins, and offers complications for glanceable access.
